DESCRIPTION:The Carpenters Christian Church Medical Ministry will be having a Living Will Fair on Tuesday\, March 21st at 6:30 in the gym at TCCC.\n\nWe will have the forms ready to complete and go over it section by section\, answering questions as we go. You can come just to get information and take the form home OR complete it that evening and it will be notarized then. The form also allows you to appoint a healthcare surrogate to make decisions if you're unable to\, as well as designate you're an organ donor if you so choose.\n\nFor those who use Ephraim McDowell Health systems\, we can make a copy and get it scanned into your medical record if you'd like!\n\nThis Fair is open to the community so let your friends and family know too!
